Center for Disease Control (CDC). Persons residing in Landis Place on King, Steeple View Loft or any Downtowners member may come to the clinic being held at

Landis Place on King in the Conexión Meeting Room

Friday, October 11 from 9 am – 1 pm.

• No registration is required.
  - You must bring a copy of your health insurance card for billing purposes.
  - An Immunization Consent form is also required. You can also get a copy of the form at either the LPK or Downtowners/SVL office. Please complete ahead of time and bring with you to the clinic.
